Personal support for planned journeys, arrivals and transitions.

A journey can be basically well planned and still need support: at arrival, during transitions, with luggage, orientation or feedback to an approved contact person.

Urban Roots reviews whether personal or organisational support is sensible, feasible and clearly definable in the specific case.

Preparation

Arrange routes, meeting points, contact options and open questions before the journey or arrival.

Accompaniment of a defined section

Accompany one clearly agreed part of a journey or transition, once role and boundaries have been clarified.

Orientation and transitions

Coordinate arrival, onward movement, accommodation, luggage or contact persons.

Feedback

After an agreed section, provide short feedback to approved contact persons.

Sensitive information

Please do not send diagnoses, medication plans, care information, ID documents, passwords or unnecessary private details in the first step.
